| Service(s) description | Roots is a Prevention and Early Intervention (PEI) partner for mental health and wellness support for children, youth and families. Roots will provide all of the navigation services for families at the clients home, school, various community sites, and gathering areas in the proximity of the Sobrante Park neighborhood. |
| Program impact | During face-to-face encounters, care plans, client progress and outcomes are discussed to provide the most meaningful array of services and supports to achieve and maintain stability. Adding Family Navigation to our existing array of resources and services for individuals has tremendous potential to contribute to the overall stability of prioritized clients. Our comprehensive intake, assessment and case conferencing processes specifically address the holistic needs of individuals as they progress from crisis to stability and enable our team to most efficiently utilize available resources. As such, we are well-positioned to utilize these tools and processes to prioritize those for whom prevention and early intervention family navigation will achieve the greatest impact. We welcome the opportunity to collaborate with HCSA/BHCS to meet the Family Coaching needs and make the greatest collective impact in Sobrante Park. |
