Program Description | For this scope of work, we are proposing to engage two distinct target groups:
Priority Group A: Juniors and Seniors at Madison Park Academy and their families. Twenty-four youth, participating in Career Readiness (Category 3) at Madison Park Academy through our subcontracting partner, Higher Ground, and their families will receive Family Navigation Services from Roots (Category 1) at the family’s home, in the community, at the school, or at our location(s); and will participate in neighborhood improvement activities/events (Category 2). We are proposing to pilot this model that connects the school to the community and the community to the school by supporting a cohort of students with career readiness and exploration, family navigation, and opportunities for civic engagement - within and outside of the school setting.
Priority Group B: Individuals at high risk and their families. Through Roots’ outreach in the community, and in collaboration with our partners, we will seek to identify sixteen (16) individuals at high risk for violence and/or behavioral health conditions to receive navigation, including family navigation and/or reunification (Category 1), and participate in neighborhood improvement activities/events (Category 2). Should these individuals require workforce training or employment opportunities, we will connect them to outside employment, or to apprenticeships at Roots and/or Planting Justice. Consistent with our own mission, we would seek to maximize the impact of this opportunity by identifying and providing prevention and early intervention services to our community’s most marginalized. |
