Program DescriptionIn 1992 La ClĂ­nica, in collaboration with the East Bay Agency for Children and the Oakland Unified School District, opened the Hawthorne Health Center as one of the first school-based health centers in Alameda County. We serve students from World and Achieve Academies (formerly Hawthorne Elementary), Urban Promise Academy (UPA) Middle School, and children from the wider Fruitvale community. We offer primary care medical services, health education, and behavioral health services. We see patients from 3-18 years old and all services are provided without charge to students. Services include: Primary care medical services for children and adolescents ages 3-18 Physical exams Immunizations Laboratory testing Prescriptions Nutrition and fitness promotion Management of chronic illnesses Urgent care Family planning services Birth control and safe sex education Diagnosis and treatment of sexually-transmitted and other infections Behavioral health care services Brief counseling Crisis intervention Case management linkage & referrals Counseling support/education groups Peer health education Referrals to specialty care as needed
